If the couples in the 20s and middle ages have a regular coexistence, one of these 5-6 couples becomes pregnant each month. This means that at the end of 1 year, 85% of couples become pregnant. If infertility tests are normal for both couples, the proposed assisted reproductive technique is IUI. It is a simple inexpensive method according to IVF (IVF treatment), so it is the first application of ART (assisted reproduction technique).
It is a simple infertility treatment in which the washed and prepared sperm is put into the uterine cavity with the help of a thin catheter. IUI is easier than IVF treatment. It is an easy and inexpensive assisted reproductive technique according to IVF which is applied after in-drug treatment or in combination with IVF. However, it should not be underestimated that its effectiveness is minimal compared to IVF.
Which groups of patients is recommended for IUI ?
• Couples with moderate infertility problems
• Unexplained infertility,
• Low sperm count,
• Reduced sperm motility, except for patients with mild to moderate endometriosis;
• Poor cervical factor,
• Cervical scar tissue,
• It is applied in male patients with ejaculation disorder.
IUI in women with ovulatory irregularity, mild to moderate spermiogram defect in the male factor, cervical mucus is applied to women with less than ideal. In order to apply the vaccination, the woman should have open, fallopian tubes, uterine cavity smooth and ovulatory.
Vaccination is useless in the serious male factor present in low sperm count and sperm deformity. Likewise, the tubs are severely damaged and will not work in women who are closed.
Step by Step IUI treatment:
In natural reproduction, eggs are taken by the tubes that are ejected from the ovaries. Sperms swim through the uterus through the cervical canal and reach a distance of 8-10 cm. This is about 10% of the total volume. If all goes well, the egg is fertilized by sperm and the embryo is formed.
Stages of IUI
1-Stimulation of the ovaries
2- Monitoring of follicles by ultrasound
3-Ovulation (egg disposal)
4-Vaccination (sperm into the uterus via catheter)
5-Pregnancy Test
In ovulation the ovaries are strengthened by oral medication, followed by egg development and endometrial thickness ultrasound. When 2 or 3 eggs reach the desired size, hCG injection is performed to trigger ovulation. After a certain time after injection, a fresh sperm sample is taken on the day of ovulation and washed in the laboratory and removed from the seminal fluid. At the end of the washing, fast motile sperm are concentrated and concentrated together. This sample is applied to the uterine cavity through a thin catheter. High quality sperm group is delivered to the egg without having to cross the cervix and uterus. It is as easy as a painless and PAP smear procedure for the patient.
Pregnancy test is positive at the second week of the procedure and it can be seen by ultrasound at the third week.
In the IUI administered cycle, the patient's conception potential is maximized. Generally, the ovaries are stimulated by high quality and multiple oocytes are obtained and brought together with a large number of sperm.
IVF is the treatment step in which the couples who cannot get pregnant during the vaccination phase are directed.
Vaccination is much more successful than the male factor in the middle and mild male factor. In serious male factor, IVF is a better option.
The total chance of success in the 3 IUI cycles is 40-55%. It is recommended that the couple be directed to IVF at this stage if the success was not achieved despite 4 cycles of grafting.
